The purpose of the DOT history and physical examination is to detect the presence of physical, mental, or organic conditions of such a character and extent as to affect the driver's ability to operate a commercial motor vehicle (CMV) safely.

A Department of Transportation (DOT) physical examination is conducted by a certified "medical examiner." The term includes, but is not limited to, doctors of medicine (MD), doctors of osteopathy (DO), physician assistants (PA), advanced practice nurses (APN), and doctors of chiropractic (DC).

A DOT physical examination is required  for any person carrying a commercial driver's license (CDL) and drives a truck, bus drivers, and CMV drivers.

Our office is here to take excellent care of you and your employees as we have two National Registry Certified Medical Examiners.  Dr. Christy Diaz, chiropractic physician, completed training in 2013 and is currently accepting new patients.  Located in the heart of Bellevue, near the intersection of Old Hickory Boulevard and SR-70, our office has been serving our community for more than 25 years with a variety of services.  We are excited to now provide certified medical examinations for CMV drivers.

We offer same day appointments most days of the week. We offer flexible scheduling as we know how important time is to drivers.  It is our goal to not only to provide excellent medical care, but to keep your drivers on the road.  We understand the rules and guidelines and we will do our best to keep your employees driving.
